  To assess the prevalence of transfusion-mediated viral infections in multi-transfused thalassemics in Bangladesh, forty-two thalassemic children (Male = 24, Female = 18) were recruited. All children were less than twelve years of age. Seromarkers for hepatitis B virus (HBV), hepatitis C virus (HCV) and human immunodeficiency virus (HIV) were studied in these children prior to initiate blood transfusion and after they had received an average of 17.0±1.0 blood transfusions over a ten-month study period. The HBV and HCV markers were significantly higher in post-transfused subjects as compared to their pre-transfusion levels (HBsAg: 19.0 vs. 7.1%, p = 0.021 and anti-HCV: 16.7 vs. 2.4%, p<0.001). None of the thalassemic children was positive for HIV before or after transfusion. The serum total bilirubin, alanine aminotransferase, aspartate aminotransferase and alkaline phosphatase levels also were significantly higher (p = 0.001, <0.001, <0.001 and <0.001, respectively) in post-transfused patients. Thus, HBV and HCV infections are major problems in multi-transfused thalassemic children in Bangladesh.

  Angiotensin-I Converting Enzyme (ACE), a major component of the Rennin-Angiotensin System (RAS) regulates the renal hemodymamics of our body. An insertion/deletion (I/D) polymorphism of ACE gene has been implicated in kidney disease, hypertension and cardiac function. In the present study, we investigated I/D polymorphism of ACE gene in 125 subjects of both sexes, consisting of 66 normal controls and 59 type 2 diabetic patients with nephropathy. The allele and genotype frequency were significantly different between the patients with diabetic nephropathy and control groups (p<0.001). In the nephropathic patients, ID and DD genotypes were present in 37.2 and 40.6% of patients respectively as compared to 42.4 and 21.2% in those of control groups. The D allele was present in 55.5% of the nephropathic patients and 44.4% of the normal controls. Therefore, percentage of DD genotype and D allele was significantly higher in nephropathic patients compared to normal controls.

  Liver function tests (LFTs) are almost widely used as indicators of hepatobiliary diseases. Activities of alkaline phosphatase (ALP), alanine aminotransferase (ALT), asparate aminotransferase (AST) and γ-glutamyl transpeptidase (GGT) as well as total serum bilirubin have been determined in the serum of 68 healthy volunteers (as control) and 184 hospitalized patients suffering from various hepatobiliary diseases. The total serum bilirubin levels were increased significantly (P<0.000) in obstructive jaundice and acute hepatitis patients. In both patient`s bilirubin elevation were found about 12 times higher than normal. Where as a moderately significant (P<0.001) elevated level of serum bilirubin up to 4.2 times higher than the normal was found in all chronic hepatitis, patients. Alkaline phosphatase (ALP) was found significantly (P<0.000) high about 4 times greater than the normal serum level, in obstructive jaundice. Both transaminase (ALT and AST) were dramatically and significantly (P<0.000) increased about 48 times higher than in acute hepatitis patients. The ALT/AST ratio was greater than one. In obstructive jaundice and chronic hepatitis patient`s transaminase (ALT and AST) were only mildly elevated (P<0.001). γ-glutamyl transpeptidase were significantly (P<0.000) elevated in patients with obstructive jaundice. However, in acute and chronic hepatitis the GGT level were normal or mildly high. All of them were referred for histological study of liver (liver biopsy).
