  Aims  In populations at high risk of gestational diabetes mellitus (GDM), screening every pregnant woman by an oral glucose tolerance test (OGTT) is very demanding. The aim of this study was to determine the value of the fasting capillary glucose (FCG) as a screening test for GDM.

Methods  FCG was measured by a plasma-correlated glucometer in 1465 pregnant women who underwent a one-step diagnostic 75-g OGTT for universal screening of GDM.

Results  One hundred and ninety-six (13.4%) women had GDM as defined by the criteria of the American Diabetes Association. The area under the receiver operating characteristic curve (AUC) of the FCG was 0.83 (95% confidence interval 0.80-0.86). A FCG threshold of 4.7 mmol/l (at an acceptable sensitivity of 86.0%) independently could rule-out GDM in 731 (49.9%) women, while the FCG could rule-in GDM (100% specificity) in 16 (1.1%) additional women; therefore, approximately half of the women would not need to continue with the cumbersome OGTT.

Conclusions  Screening using a FCG significantly reduces the number of OGTTs needed for the diagnosis of GDM. Wider assessment, particularly in low-risk populations, would confirm the potential value of the FCG as a screening test for GDM.

  In the present study, it was envisaged to prepare three polyherbal formulations (F1, F2 and F3) containing leaves of Solanum xanthocarpum and Andrographis paniculata, rhizomes of Curcuma longa and stem of Tinospora cordifolia in varying ratio and evaluating the polyherbal formulations for the antipyretic and antinociceptive activities. The antipyretic activity of methanol extracts (MF1, MF2 and MF3) and aqueous extracts (AF1, AF2 and AF3) of polyherbal formulation were studied in Brewer’s yeast induced pyrexia in mice. The antinociceptive activity of methanol and aqueous extracts of polyherbal formulation were studied using Eddy’s hot plate method and tail flick method in mice. The polyherbal formulation of all extracts showed significant reduction in the elevated body temperature of rat which was compared with standard paracetamol. The extract of polyherbal formulation produced significant increase in the reaction time by Eddy’s hot plate method and tail flick method in mice which was compared with standard morphine sulphate. From these results it may be concluded that AF1 formulation demonstrated maximum significant antipyretic and antinociceptive activities that might be due to combined effect of active constituents present in all plant material.

  Fifty proteolytic bacterial cultures were screened for their potential to degrade chicken feather. Eight strains which degraded chicken feather within 24 h were further evaluated for degradation of surrogate yeast prion protein, Sup35 NM. Prion degradation was studied by congo red binding assay and a strain of Pseudomonas aeruginosa KS-1 was selected for further evaluation as it exhibited maximal shift in absorbance of congo red from 0.268 to 0.926. Keratinase from this strain was purified using Q-sepharose chromatography with 60% recovery and a purification fold of 2.24. It was identified as a 33 kDa monomeric protein having a pH and temperature optima of 7 and 50°C, respectively. It was stable over a broad range of pH ranging from pH 2-11 and was highly thermostable with a t1/2 of >2 h at 80°C. It was a serine, thiol activated protease with two fold increase in activity in presence of β-mercaptoethanol. It also hydrolyzed a range of complex protein substrates including casein, feather, fibrin. Amidolytic activity on synthetic substrates revealed that it efficiently cleaved phenylalanine and arginine. It hydrolyzed insulin B-chain at cystein, glycine, histidine, phenylalanine and leucine as revealed by ESI-MS analysis.
