  In silico biology, including computers, databases, methods and algorithms are used for statistically analyzing the data to achieve the extract information and to identify the relationships between these data sets. This study aimed to provide alternative and new drugs that connect to DNA by using in vivo, in silico and find the best option from the newly designed series of drugs. Out of 143 DNA dodecamer crystal structures available in PDB, we have selected three structures and retrieved from the Protein Data Bank (PDB), with minor/major groove or both groove biding mode. Eighteen experimental ligands were docked using AutoDock 4.3 program into the active sites of selected drug-DNA structures. We docked our experimental ligands into three modeled DNA structured and compared those to internal evaluated ligands (drug compounds include ditercalinium, adriamycin and propamidine) and observed the same binding sites in experimental ligands in comparison to internal ligands. Docking results of 6m and 6p compounds into oligonucleotide in comparison to adrimycin and propamidine showed almost same binding fashion. Our results display that the most plausible mode of action of these experimental drugs as DNA binding agents is through intercalation of AT base pairs-linker chain and azole-minor groove. Accordingly, other DNA-drug crystal structures can be used as good patterns for further improvements using in silico and structure-based drug design methods.

  The aim of this study was to compare the analgesic effects of Femoral Nerve Block (FNB) with intravenous Morphine Sulfate in different type of femoral fractures in the emergency department. In this randomized not blind clinical trial all patients aged 15-60 diagnosed as femoral fractures with neurovascular and hemodynamic stability were assessed for eligibility. Patients randomized into two groups to receive either FNB blindly with Lidocaine 1% or intravenous Morphine Sulfate 0.1 mg kg–1 the patients`pain score recorded before and 5, 30 and 60 min after intervention using visual analogue scale. The fracture type ( neck, inter-trochanteric or shaft of femur) was recorded for all patients. Present results showed that the pain score before and 5 min after intervention was not significantly different between two groups (p = 0.65 and p = 0.77, respectively) but 30 and 60 min after intervention the pain score in FNB group was significantly lower than that of the Morphine group (p = 0.002 and p = 0.001, respectively). Comparing the pain score at minute 30, as the primary outcome, was not significantly different in neck of femur fracture (p = 0.76), while these differences were significant in inter-trochanteric and shaft of femur fracture (p = 0.00 and p = 0.013, respectively). Present results showed that the blind FNB could be used as an effective analgesic treatment in patients with femoral fracture.

  This study aimed to design a reliable homology model of human histamine H1 and H4 receptors (hH1R, hH4R), that would guide future biochemical and genetic efforts in its evaluation as a potential therapeutic target. Furthermore, these accurate models could aid in the structure-based inhibitor design for antagonists against the histamine H1 and H4 receptors. The homologous protein sequences of histamine receptors were retrieved from the NCBI REFSEQ which by using the sequence alignment program ClustalW alignment of the human histamine receptors sequence with Bovine Rhodopsin was conducted to locate the homology aligned regions. The present study found that Asp107 and Asn198 are in favorable positions for anchoring histamine. Identification of novel interaction sites for antagonist binding mutational data suggest a crucial role for Asp107, Trp158, Phe 432 and Phe 435 in antagonist binding. This study identified several novel amino acids at the binding site. Binding mode analysis of known H1 antagonists four known H1 antagonists (mepyramine, acrivastine, desloratadine, loratadine) were docked successfully to the binding site of the hH1R model by FlexiDock. The ligand used for optimizing the receptor model, the pharmacophore constraints and the different scoring functions applied in high throughput docking had all significant effect on the results. This research identified 16 compounds with 7 significant H4 activities representing an overall hit rate of 5.2%. To the best of our knowledge, this is one of the largest structure-based virtual screenings, where the virtual hits were confirmed by an in vitro assay. Moreover, this is the first structure-based drug design study reported on the hH4R. After the virtual screening, we identified several novel ligands with significant H4 affinity. These scaffolds can serve as starting points in the development of potent and selective H4 ligands in future.

  One of the most common cancers in the world, especially in underdeveloped countries, is Gastric Cancer (GC) which kills thousands of people annually. It is also prevalent in our country, Iran and according to statistics, it is the most common cancer in men. Since, the relationship between expression of CD34 cell surface markers and many cancers has been approved in the present study, the marker expression association with GC is studied. In this study, a total of 47 paraffin blocks of patients with GC referring to Imam Khomeini hospital in Ahvaz, during 2003-2013 were used. The mean age of 47 patients, including, 35 men (74.5%) and 12 women, was 60±16.05 years old ranging from 24-84. The mean tumor size was 34.5 ranging from 2-12 cm. More than 71% of tumors were intestinal type and the depth of more than 74% of them was in T3 phase. Incidence of marker was positive in only 8 patients (17%) and in rest 39 was negative. The CD34 expression was significantly correlated only with the tumor type (p = 0.00), degree of malignancy (p = 0.00) and neural invasion (p = 0.01). In this study, a positive correlation was observed between the marker expression and three factors of tumor type, degree of differentiation and neural invasion. It was also concluded that due to the negative CD34 expression in 83% of samples, the absence of positive CD34 cells in tumor tissue can be considered as a marker in GC, although it is recommended to do further research with more samples in this context.

  Fine Needle Aspiration (FNA) is a relatively painless, safe and inexpensive diagnostic method, which is a widely accepted for the diagnosis of salivary glands neoplastic and inflammatory lesions before surgery. The aim of this study was to evaluate the diagnostic value of FNA in the diagnosis of salivary gland tumors nature compared with the histopathological results after mass excision. This is a cross-sectional study, where since April 2009 to March 2012, all patients complaining about salivary gland tumors referred to the ear, nose and throat ward of Ahvaz Imam Khomeini hospital and undergone FNA and biopsy have been studied. Then, the results of FNA were compared with pathology results after mass excision, as well as the diagnostic accuracy of FNA was calculated. In this study, 160 patients were studied including 88 females (55%) and 72 males (45%). Most patients were in the age group of 31-40 years and lowest in patients aged over 80 years. The mean age of patients was 41.7±18.9 with a minimum 3 years and the maximum was 85 years old. The FNA sensitivity in salivary gland tumor to pathological findings after mass excision was calculated 92.3% and its characteristics 99%. The positive predictive value was 96% and negative predicted value 97.9%. As shown in this study, FNA sensitivity has a high specificity and diagnostic value in the detection of the salivary gland lesions. Given that it is an inexpensive, safe and reliable method, it is recommended to be used more widely.

  Non-Alcoholic Fatty Liver Disease (NAFLD) is a very prevalent condition determined by the infiltration of fat of liver cells. This study was conducted to evaluate the correlation between NAFLD and Carotid Intima-Media Thickness (CIMT) in patient with type II diabetes. This case-control study included three groups, including those with type II diabetes without NAFLD (n = 85), those with type II diabetes and NAFLD (n = 85) and non-diabetic individuals with only NAFLD (n = 85). The CIMT and the grading of the NAFLD were determined using an ultrasound B-mode. There was no significant difference between the case and control groups in term of overall mean of age (57±10 years vs. 56.9±8 years, respectively). Considering the two factors (diabetes and NAFLD) effect, with adjustment for age and gender as a confounding effect of diabetes (p = 0.01) and NAFLD (p = 0.022) on CIMT was significant. The mean right CIMT, left CIMT and overall CIMT in three different study groups of diabetic patients, according to various NAFLD grading showed no significant difference. Our findings showed a considerable association between NAFLD and increased CIMT, in which this association is not affected by the severity of fatty liver. So, immediate ultrasound screening and treatment for the patients with NAFLD are recommended to prevent CVD complications such as atherosclerosis considering early stages of fatty liver disease.

  As our knowledge no report was given about effect of leukemia inhibitor factor on sperm motility and survival rate of asthenospermic infertile men. That's why this study was decided to review the effects of Leukemia Inhibitor Factor (LIF) with different concentrations of 0, 3, 5, 10, 50 ng mL-1 on motility and survival rate of asthenospermic infertile men. Semen samples of 15 asthenospermic men who referred to IVF unit of Imam Khomeini Hospital, Ahvaz, Iran were collected and put in incubator under condition 5% CO2 in air at 37°C for 45-30 min then total sperm count was done first, then calculate the motile sperm with a degree a and b. In this study, we evaluated only samples that had sperm motility percent of more than 30%. After that time from every sample about 10 μL removed and culture in different media. Every drop was evaluated 6, 24, 48 h after cultured of sperm in it for motility and survival rate of sperm. Statistical analysis shows that the forward motility of sperm cultured for 6 h in media with and without of LIF is not significant (p>0.05) but after 24 h the forward motility and survival rate of sperm cultured in media with 10 and 50 ng mL-1 of LIF significantly increased (p<0.05) and after 48 h the forward motility and as well as survival rate of sperm cultured in media with 50 ng mL-1 of LIF significantly increased (p<0.05). The conclusion from this study is that certain concentrations of leukemia inhibitor factor can increase the motility and survival rate of sperm.

  The present research was conducted in order to investigate the effects of right-unilateral lesion of substantia nigra neurons by means of Ibotonic acid, a cholinergic-selective neurotoxin, on hematological parameters in rats. Thirty male Wistar rats weighing 200±50 g at the start of the experiment were used. The substantia nigra was right-unilateral lesioned by stereotaxic microinjections of 8 μg (free base) ibotonic acid, dissolved in 4 μL physiological saline containing 0.1% ascorbic acid, administered through the Hamilton microsyringe over 4.50 min. Seven days after neurosurgery, we assessed the total number of White Blood Cells (WBC), the total number of Red Blood Cells (RBC), Red cell Distribution (RDW), platelet and hemoglobin level and the erythrocyte indexes, Mean Cell Volume (MCV), Mean Cell Hemoglobin (MCH), Mean Cell Hemoglobin Concentration (MCHC). Ibotonic acid treatment induced a highly significantly decrease of white blood cells (p<0.001), followed by significant decrease in red blood cells (p = 0.02) and hemoglobin level (p = 0.03) comparative with sham-operated rats. Also, in the ibotonic acid-lesioned rats the erythrocyte indexes, Mean Cell Volume (MCV) (p = 0.04); Mean Cell Hemoglobin (MCH) (p = 0.02)) were significantly decreased comparative with sham-operated rats. By contrast, in the ibotonic acid-lesioned rat's platelet (0.34), Mean Cell Hemoglobin Concentration (MCHC) (p = 0.41) and Red blood cell Distribution Width (RDW) (p = 0.03) were significantly enhanced comparative with sham-operated rats. On the whole, the obtained data indicate the important role of the central nerves system in the regulation of erythrocyte dynamics.

  The objective of this study was to evaluate whether cryopreservation of small volume of sample (sperm+cryoprotectant) was feasible using open pulled straw and also compared the outcomes of open pulled straw and conventional straw as carrier for normal human sperm cryopreservation. Semen samples were obtained from 10 men undergoing evaluation for infertility after 3-4 days of abstinence. Washed normal sperm samples were divided into three aliquots as follows: (1) Fresh; (2) cryopreserved in open pulled straw and (3) crypreserved in conventional straw. In order to do cryopreservation of sperm in open pulled straw first washed normal sperm samples were mixed with equal volume of test yolk buffer and 12% v/v glycerol, later on 3-4 μL from the prepared mixture was loaded in each straw by using syringe. The loaded straws were plunged into liquid nitrogen and after 3 months recovered and thawed. Each straw was emptied of their fluid content in drop of 10 μL medium covered with mineral oil. Motility of vitrified-thawing sperm was assessed by using inverted microscope. The results show as percent progress motility±SD and the p<0.05 were suggested as significant. The percent progress motility±SD of fresh sperm was evaluated as follow in study groups: For fresh group was evaluated as 59.2±7.6; the value of 37.5±8.2 in cryopreserved sperm in open pulled straws group; and value of 26.3±6.4 for conventional straw group, respectively. Statistical analysis shows that the difference between cryopreserved sperm in open pulled straws and conventional straw groups is significant (p = 0.001). Because of the significant difference between cryopreserved sperm in open pulled straws and conventional straw groups, we concluded that vitrification of human sperm is feasible using open pulled straw. The results of this study shows that open pulled straw could be a good carrier for cryopreservation of small volume of normal human sperm.

  We attempt to review all databases from which data on medical malpractices and complaints on Iranian health system were presented. We systematically searched PubMed, Medline, four national databases and hand-searched three relevant journals from 1991 to 2011. After quality appraisal, data was extracted from the literature and analyzed using qualitative synthesis approach. Twenty-four reports on the medical malpractice in various medical specialties, dentistry and pharmacy were reviewed. A total of 8394 complaints out of 16584 cases were found related to medical malpractice in Iran from 1991-2011. The highest number of medical malpractice reported, were from the capital city following by eastern north part and central part of Iran. The highest risk of medical liability lawsuits were observed in obstetrics and gynecology, orthopedy, general surgery and ophthalmology. Studies only focused on some elements of the problem and rarely on process-related or systematic causes of malpractice and medical errors. There is substantial variation in the likelihood of malpractice suits across specialties. Despite considerable financial impacts of healthcare fraud and malpractice, this aspect of the problem had received even less attention.

  Carpal Tunnel Syndrome (CTS) is a common problem that is caused by pressure on a nerve in the wrist and is associated with various factors. We compared Cross-Sectional Area (CSA) of the inlet and outlet in CTS patients and healthy controls and define the best cut off point of CSA and assess the utility of US in the diagnosis of CTS patients as well. This cross-sectional, age-group-matched case-control study was performed on 39 patients aged 42-65 years with electro-physiologically confirmed idiopathic CTS and 35 healthy controls from Golestan Hospital, Ahvaz, Iran from Jan 2013 to Feb 2014. Neurological and electro-diagnostic tests for patients were performed by an expert neurologist as the gold standard diagnostic test for CTS cases. Hundred wrists were evaluated. Electromyography (EMG) showed that 16 (32%) cases were mild, 17 (34%) were moderate and 17 (34%) were severe. The results showed that the CSA of carpal tunnel inlet, carpal tunnel outlet and AP diameter were significantly different between the two groups (p<0.001, p = 0.009, respectively). The sensitivity and specificity of carpal tunnel inlet were 92 and 96%, for carpal tunnel outlet were 92 and 92%, for AP diameter were 64 and 58%. longitudinal evaluations are necessary to better know the utility of ultrasound for diagnosing CTS. The accuracy of diagnosis increases in parallel with the increase in the stages and severity of CTS. Therefore, patients with severe CTS would not need the more uncomfortable, costly and invasive techniques.

  Vitrification is the commonly used method for long-term storage of pre-implantation mammalian embryos. It is an essential part of assisted reproductive technologies. The re-expansion rate, pregnancy and birth rate of vitrified blastocysts using CPS were compared with OPS and Conventional Straw. Female NMRI mice were injected with Gonadotrophins in order induce them for super ovulation. At that time the mice were sacrified by cervical dislocation and dissected of mouse abdomen. The uterine horns were existed blastocysts were collected in PBS and randomly allocated to four groups: vitrification in CPS, conventional straw, OPS and untreated controls. The vitrification solution was EFS40%. After storage for 1 month in liquid nitrogen, the blastocysts were thawed in 0.5 M sucrose for in vitro culture in M16 medium. After 6 h of culture, the numbers of expanded blastocysts was recorded and ready for transfer to uterus of pseudo pregnant mouse. The re-expansion rate of the CPS group (72.1%) was significantly higher (p<0.05) than OPS (52.55) and C.S. (38.6%) groups. The pregnancy (70%) and birth rate (45%) of blastocysts in CPS were similar to those of fresh blastocysts (80% and 45.5%) and the pregnancy (10%) and birth rate (5.1%) in Conventional Straws lower than OPS (20 and 7.5%), but were not significantly different. Mouse blastocysts vitrified using CPS had a better result compared with OPS and Conventional Straw. The value of CPS for vitrification of blastocysts may also merit investigation.

  This study determined the distribution of a specific group of Simple Sequence Repeats (SSRs), in genome sequences of 7 chromosomes (Shigella flexneri 2a str 301 and 2457 T, Shigella sonnei, E. coli K12, M. tuberculosis, M. leprae and S. saprophytycus) have downloaded from the GenBank database for identifying abundance, distribution and composition of SSRs. The data obtained in the present study show that: (i) Tandem repeats are widely distributed throughout the genomes. (ii) SSRs are differentially distributed among coding and non-coding regions in investigated Shigella genomes. (iii) Total frequency of SSRs in non-coding regions is higher than coding regions. (iv) In all investigated chromosomes ratio of Tri-nucleotide SSRs are much higher than randomized genomes and Di nucleotide SSRs are lower. (v) Ratio of total and mono-nucleotide SSRs in real genome is higher than randomized genomes in E. coli K12, Sh. flexneri str 301 and S. saprophyticus, while it is lower in Sh. flexneri str 2457T, Sh. sonnei and M. tuberculosis and it is approximately same in M. leprae. (vi) Frequency of codon repetitions are vary considerably depending on the type of encoded amino acid.

  This study aimed to assess a meta-analysis of the association of XRCC1 polymorphisms with the risk of various ophthalmologic diseases in Asian population. This meta-analysis was performed by critically reviewing reveals 38 studies involving 1373 cases and 1745 controls. Among all the eligible studies, one focused on Arg194Trp polymorphism, nine described the Arg399Gln and no article investigated on Arg280His. There was a large between-study heterogeneity in ORs of individual studies of the dominant model (χ2 = 74.18, I2 =58.9%, p = 0.013) and the additive (χ2 = 56.18, I2 = 41.4%, p = 0.091) models, but a moderate heterogeneity in the recessive model (χ2 = 72.27, I2=78.8%, p = 0.000) was observed. So, we pooled the results using the random-effect analysis and found that Arg399Gln has a weak relation with ophthalmologic disease in the recessive (OR = 0.96, 95% CI: 0.64-1.44), the dominant (OR = 1.05, 95% CI: 0.82-1.33) and the additive (OR = 1.15, 95% CI: 0.77-1.70) and models. The present meta-analysis correspondingly shows that comprising diverse population is very important since susceptibility loci might vary indifferent ethnic groups. To ratify our findings, widespread studies with enlarged sample size and various populations are essential to explain the role of all polymorphism of XRCC1 genes in the pathogenesis of ophthalmologic diseases. Finally, our meta-analysis showed Arg399Gln variant was not associated with increased ophthalmologic diseases risk via dominant and recessive modes among Asian population.

  The aim of this study was to evaluate the benefits, impact and overall efficacy of tonsillectomy or adenotonsillectomy on quality of life in patients with recurrent, chronic tonsillitis or adenotonsillar hypertrophy before and after surgery in adults and children. This stusy is a multi-center retrospective study of all adults and children, who had undergone tonsillectomy in adults (14 years or older) and adenotonsillectomy in children for chronic, recurrent tonsillitis or adenotonsillar hypertrophy between September 2003-2008 in Ahvaz Imam and Apadana Hospitals. All 812 patients were asked by questionnaire to compare their symptoms 6 months period before and after tonsillectomy or adenotonsillectomy. Outcome measures included the frequency of tonsillitis per year, days off work (or school), doctor visits and feelings of well-being. A total of 812 patients (81 adults and 731 children) were enrolled in the study. The age of the patients ranged from 3-42 years. In adults group, there were 34 men and 47 women. In child group, there were 325 boys and 406 girls. The mean age of the children patients was 9 years and adults 26 years. The mean duration of symptoms was 4 years. The mean frequency of tonsillitis per annum, days off work (or school), doctor visits decreased postoperatively. We concluded that tonsillectomy in adult and adenotonsillectomy in children is benefit for patients with adenotonsillar hypertrophy and/or recurrent, chronic tonsillitis and results in significant improvement in overall quality of life, physical health and general well-being.
